 Adrenal Crisis Overview

An adrenal crisis is a medical emergency that can cause life-threatening complications such as shock or kidney failure if not treated. A crisis is usually set off when affected individuals are under stress during an accident, trauma, surgery, or severe infection. An adrenal crisis is characterized by an acute change in physiologic status, and quick progression from nonspecific symptoms of fatigue, weakness, nausea, vomiting, abdominal pain, back pain, diarrhea, dizziness, hypotension, syncope, and eventually to obtundation, metabolic encephalopathy, and shock.

An Adrenal crisis most often occurs in a patient with known adrenal insufficiency, either primary or secondary. Primary adrenal insufficiency (Addison Disease) is characterized by low cortisol and high ACTH from the destruction of the adrenal glands. Secondary adrenal insufficiency is characterized by low cortisol and low to normal ACTH caused by decreased ACTH secretion from the pituitary.
